Pengaruh imunoterapi sebagai upaya terapeutik wanita dengan endometriosis: Kajian Naratif

Introduction: Endometriosis is one of the most common gynaecological and systemic diseases, with a remarkable immune background. Patients suffer from pelvic pain and decreased fertility. Due to the different immune components, immunotherapy approaches may be necessary. In endometriosis, the immune system cell fraction shift is well known. In addition, hypoxia and inflammation cause a compromised immune response. Removal of endometriosis has a therapeutic effect, normalizes immune disorders, and remains the most effective causative treatment for pain and infertility. Treatment of endometriosis with immunotherapy is promising due to the significant contribution of immunological factors and the limitations of current treatment methods. The purpose of this study was to determine the effect of immunotherapy in improving fertility in patients with endometriosis. This research method uses a literature review and narrative method by collecting research results related to immunotherapy and endometriosis. Data sources were obtained from various international journals indexed by Scopus from 2018 to 2023. Results: Immunotherapy affects improving fertility in patients with endometriosis. Conclusion: Immunotherapy can improve the peritoneal immune microenvironment and inhibit the development of endometriosis lesions, working synergistically with hormonal therapy to fundamentally relieve disturbing symptoms and improve fertility in patients with endometriosis.
Pendahuluan: endometriosis merupakan salah satu penyakit ginekologi dan sistemik yang paling umum, dengan latar belakang imun yang luar biasa. Pasien menderita nyeri panggul dan penurunan kesuburan. Dikarenakan komponen imun yang berbeda, pendekatan imunoterapi mungkin menjadi penting di masa mendatang. Pada endometriosis, pergeseran fraksi sel sistem imun sudah diketahui dengan baik. Selain itu, hipoksia bersamaan dengan peradangan menyebabkan respons imun terganggu. Pengangkatan endometriosis memiliki efek terapeutik, menormalkan gangguan imun, dan tetap menjadi pengobatan kausatif yang paling efektif dalam hal nyeri dan infertilitas. Pengobatan endometriosis melalui imunoterapi cukup menjanjikan karena kontribusi signifikan faktor imunologi dan keterbatasan metode pengobatan saat ini. Tujuan dalam penelitian ini yaitu untuk mengetahui pengaruh imunoterapi dalam meningkatkan kesuburan pasien dengan endometriosis. Metode penelitian ini menggunakan literature review dan bermetode narative dengan mengumpulkan hasil-hasil penelitian tentang terkait imunoterapi dan endometriosis. Sumber data didapat melalui berbagai jurnal internasional terindex scopus dari tahun 2018 hingga tahun 2023. Hasil: Imunoterapi berpengaruh dalam meningkatkan kesuburan pasien dengan endometriosis. Kesimpulan: Imunoterapi dapat meningkatkan lingkungan mikro imun peritoneum dan menghambat perkembangan lesi endometriosis, bekerja secara sinergis dengan terapi hormonal untuk secara mendasar meredakan gejala-gejala yang mengganggu dan meningkatkan kesuburan pada pasien dengan endometriosis.
