Physiotherapy management of cervical spondylosis with ultrasound therapy and therapeutic exercise: A case study at Hermina Bekasi Hospital

Abstract

Cervical spondylosis is a degenerative condition affecting the cervical spine, characterized by dehydration and shrinkage of the intervertebral discs, which function as shock absorbers. Another common feature is the formation of osteophytes along the vertebral margins. Physiotherapy intervention for this condition included Ultrasound therapy, free active movement, stretching, Stretching and Strengthening. This study aimed to determine the benefits and effects of providing Ultrasound, Free Active Movement, Stretching and Strengthening interventions in cases of Cervical Spondylosis. A case study method was employed with 5 treatment sessions. Outcomes measured included the Numeric Rating Scale (NRS), goniometer, Manual Muscle Testing (MMT), postural grid, and Neck Disability Index (NDI) questionnaire. After administering the intervention 5 times, the results obtained were a decrease in tenderness from 5 (T1) to 3 (T5), a decrease in motion pain from 6 (T1) to 4 (T5), spasms decreased after therapy, an increased range of motion in cervical flexion from 32° (T1) to 47° (T5), increased strength in the shoulder flexor, shoulder abductor, elbow flexor, elbow extensor, wrist flexor, and wrist extensor muscles, an improvement in posture and gait pattern, and functional activity ability increased, as indicated by decreased NDI score from 25/50% (T1) to 21/50% (T5). In conclusion, the combination of Ultrasound therapy, Free Active Movement, Stretching and Strengthening in a patient with Cervical Spondylosis can reduce pain, reduce spasms, increase the range of joint motion, increase muscle strength, improved postural alignment and functional ability.
